Correction of Nasaltip-alar complex in cleft lip palate(a new technique)

Nasal tip - Alar complex correction in unilateral Cleft Lip-Palate

Residual asymmetry of the nose after the surgical correction of cleft lip and the cleft palate is a complex deformity, which is very difficult to correct up to the satisfaction. In this deformity, not only the cleft side nasal tip-alar supporting cartilage,nasal septum are deformed and displaced but alar soft tissue also is deformed(buckled). This deformed soft tissue does not allow gaining necessary convex shape elevation of the nasal tip dome-alar region by conventional surgical correction. To achieve and retain the elevation, a strong springy elastic supporting ‘material’ is required. For this, I am using conchal cartilage autograft, harvested from the concha of the ear. The concave  surface of the cartilage graft is bent in reverse and placed on the cleft-side dome and wing of the alar cartilage. This reverse way bending gives strong,springy elastic elevation and convex shape to the soft tissue as well as to the notched alar cartilage. The technic and results, as under,are excellent.

Conchal cartilage is harvested from the ear.

Conchal cartilage graft is fixed in Collumella in between domes of alar cartilages and broad end of the cartilage bent in reversed to its natural concave shape so that its springy
convexity uplift the depressed alar soft tissue(graft is stitched to underlying soft tissue and crooked alar cartilage wing).

Double conchal cartilage grafts are placed, bent in reversed way and fixed.
